I had salvage radiation for recurrent prostate cancer in my seminal vesicle on 1/5/17 and my PSA at that time was 5.07 and as of 4/10/17 my PSa is at 7.75 is that normal and what is my next step since I thought the radiation would eliminate the tumor
I originally had cyber knife treatment in 2007 and my psa began to rise in 2016

For me the next step after salvage radiation was hormone therapy. I took eligard shots every six months and also took casodex. Before starting the casodex I had two or three rounds of radiation to my chest to help prevent breast enlargement. Hopefully you have a good medical oncologist to help you look at your options.

Thanks for the info meeting with my radiation oncologist will discuss next step, trying to get a feel for what action others have taken and results are you still o hormone therapy and has it worked for you

After two years and one month my PSA is .16. It had become non-detectable for awhile before it rose to .27 and now it is back down to .16 as of last Friday. I hope to get another year out of it, but grateful for the PSA numbers so far.

If it were successful, your PSA would go down, not up (unless you have a history of prostatitis).

Did they use a PET scan to identify the recurrence in the seminal vesicles?

Next step would be maintenance using hormone therapy. You should be talking to a medical oncologist.

Met with radiation oncologist wants to wait for my next psa test in 30 days not sure doctor is a believer in hormone therapy waiting for next psa then may move to hormone therapy my urologist w anted to wait until 10

You should be talking to a medical oncologist, NOT a radiation oncologist or a urologist.

So, no attempt was made to use any kind of PET scan to identify any distant mets before they gave you salvage radiation? They might have spared you that. And did the salvage radiation field include the pelvic lymph nodes?

I just started hormone treatment I recd 2 shots of Degarelix in my stomach and am scheduled to get a shot of Lupron in 28 days my PSA is now up to 10 and I had a Cholene c-11 pet scan which indicated I had 2 very small lymph nodes that were possible cancer to small to biopsy they were in my bowl and spine area the next step is to take Hormone treatment monitor my PSA and possibly radiate the lymph nodes - does anyone have any experience with Proton Radiation and have you heard of anyone who has had success ? with it????

Hello John..it would be very helpful to people reading your thread and posts if you would put together a short "signature" that outlines your diagnosis and your treatments so far..On the upper right of the page you will see a "my Profile" button. then "edit profile" then scroll down to signature and fill it in..

Proton treatment (at least as a primary treatment) has not proven itself worthwhile when treating prostate cancer..it seems it doesn't work any better than photon but it's much more expensive so insurance companies usually refuse to pay for it. Today, for treating a few mets, SBRT is usually used if it's deemed to be worthwhile...While the PET scan can spot mets in soft tissue, having a Bone Scan done also will light up any bone mets...PC seems to favor moving to the bones...

ADT, Hormone Therapy, comes with a heavy package of side effects that you should brace yourself for..

i guess that means that the pelvic LNs were not included in the radiation field of your salvage radiation, so it may still be a possibility for you. That might be more useful than zapping individual mets. Protons aren't used to irradiate individual mets as far as I know. SBRT is the treatment of choice for that using 1 to 3 zaps.

I hope the hormone therapy works well. Considering the beneficial outcomes of Zytiga in the STAMPEDE trial in the subgroup of recurrent patients with metastases, you might want to discuss that approach with your MO. The question is whether insurance will cover it.
