Those categories aren't based on pathology reports - that would be pointless because they are used as prognosticators of whether the primary treatment will work. For "favorable intermediate risk," the clinical (i.e., before treatment) risk factors have to be GS3+4, PSA<10, stage T1 or T2a, and less than half the biopsy cores positive.
