My decipher test results came back today so I went in to discuss. Score of .27 he (Dr Martinez) says falls within lower risk category and I should feel much more comfortable now with one therapy alone. He also mentioned active surveillance which I was not expecting. Im happy about this result but I dont think comfortable enough for AS. Wants to simply retest psa in a 2-3 months and check testosterone levels. How much weight does this decipher test carry for us? Im still sceduled for SBRT next month....

Your call, but in your place, I'd likely keep my appointment with the SBRT machine.

With your 3+4s I don't see how you would be a candidate for AS.

Frosty, I had the Decipher test, under different circumstances, as I had a PSA rise, as a 3+4, and was concerned. Score was .37, still low risk. Dr said the same thing to me, "this should make you feel more confident. My next PSA after that rose again.

I was originally a G6, was upgraded after surgery. I'm not a big fan of AS to begin with (please, no attacks, it's just me), but I would not ever consider AS as a G7(3+4).

Being ok with only one treatment, is a different matter, I think you should follow the Drs lead on that

I take all the biopsy-based genetic tests with a grain of salt. The reason is because there is considerable genetic variation from core to core, and they may have missed some more virulent spots. Decipher has not been prospectively validated on biopsy cores, and I've seen limited data on it. Certainly it's true that with your favorable intermediate risk PC, your probability of detectable metastases within 5 or 10 years is slight - but I could have told you that even without the test. The only real use for pre-treatment men is on the other side - i.e., to tell you if you have an unusually virulent type that is not yet reflected in your GS/PSA/Tstage risk level. So really, after that expensive test, you are exactly in the same position you were in before the test. You may choose to stay on AS - it is a riskier decision for men at your risk level, but not an unreasonable option. Or you may choose radical treatment. It's very much a personal decision.
