Thanks for all the positive comments and thoughts, everyone! This is the happy scenario we sought from the outset, normal testosterone with a low and stable PSA. That durable remission was always the goal. And yes, with any luck, maybe it will continue down just a bit more.

yes that is wonderful news. I am wondering with all the time on ADT, did you develop any bone loss (osteopenia) and if so, did you have any treatment? Or did your doctor prescribe some medication to avoid bone loss? I may need longer term adt so wonder about this.

Teddy, I sure did. My baseline DEXA scan showed normal bone density. After about
18 months on ADT, I had another scan showing oste
openia (T score -2.3), closely verging on osteoporosis (when T<-2.5). We started Prolia then, and boosted my calcium (1200 mg/day) and vitamin D, and weight-bearing exercise regularly. After another 2 years, the oste
openia had improved a little (T score -2.0).
When I stopped the ADT, I also stopped the Prolia (it was due about
3 months later).
Prolia is a mixed bag. I had to get a dental clearance letter before starting, due to the risk of jaw osteonecrosis. Kind of intimidating. Also, when stopping there are concerns about
fracture risk, and in fact some recommend never stopping it. I don't know what's right.
With recovered testosterone, I'm hoping my system normalizes and the bone stuff just recovers to some normal level. I'll have another DEXA scan a year from now (done every two years).
