My take is I think you are right when you say SRT may be on your horizon. It appears you have a trend, but at these low values it's unlikely anything will show on a scan imo. You are far too young to risk progression and possible mets a mere decade from now. As was mentioned those margins may not have been as clean as was reported. What I understand is that it is difficult to differentiate tissues at the apex, where you had a grade 4 component.

You say you've had digestive problems and believe me I know how that is. Frankly I see no problem waiting for another UPSA or two, but I think you'll be getting RT eventually. And if you can get an additional 10% improvement in outcome with HT then definitely give that serious consideration as well.

That's because the Cleveland Clinic nomogram had only a very small sample of men who received uPSA. In an email, Dr Tendulkar explained to me that sample size limits the validity for low PSA as well as PSA kinetics. It included post-op ultrasensitive PSA test results for only a minority of the patients (277 or 18%), and only 18 patients were treated at a PSA≤0.05 ng/ml. Zelefsky is right that the predictive validity of a nomogram is only as good as the sample size for the specific set of risk characteristics. I was hoping that the MSK database would be larger, but apparently not.

Just to be clearPrato and others..I would never advocate for someone else to do no nothing except if they were at an "advanced" age{ tho I do think you....Prato... could "get away" with that}I fully understand others temprements personalities and other differences and how that effects decision making...I get that getting away with it is not of your mind set...In my situation and I have said this before...I have knowledge of my disease...which is to lengthy to get into ...tho I have spelled it out in pieces since I have been posting here...It deals with placement of tumors...midnight calls to pathology dept where I was treated...a whole lot of dis and dat...all this and my faith in faith...creates a competence implied confidence implied competence on and on...that frees me to live freely...I may in deed and truth be riddled with cancer that has not presented with pain...and I may not...I'll play the odds...in the same way I do when I go into the ocean and hunt/chase down a 8' wave...catch that sucker vying with a gaggle of young bucks and buckess's...for that wave and utilize to my advantage every section of it
I do not need the fame or fortune of such....it is the real time of life...and the reality that I can "get away" with it at 75...so it goes with Pca I refuse to be dis-eased with any disease...Ill give up the pound of flesh for the 28 grams of spirit anytime ...any place...Lord do not tarry...I am thine...death wish?...if that makes one more comfortable...go for it...I know who and from whence I come and go...
When folks tell me "I want to be like you...kiddingly or real...I always respond right thru their eyeballs..."YOU look pretty damm good on you" They get it...they get it..." don't need no stinkin badges...take it in and surf it out.... devil smile wink
