Does/can our natural immune system kill even one cancer cell

I’ve read that cancer eventually overwhelms the immune system, but does that statement mean, before that stage is reached - there is actually a fight going on or cancer is always wearing its invisible cloak making it impossible for Killer T-cells to act on them.

It would seem some cancers "invisible cloak" works much better than others at granting immunity to T-cell attack. Other, lethal cancers are not recognized by normal T-cells.. But today, the most promising cancer research is focused on immunothreapy . Treatments like Provenge and Kaytruda are being used today with varying degrees of success..They both alter your own T-cells so they can recognize and attack cancer cells. In my opinion, it is in this line of research (immunotherapy) that a true cure for cancer will be found...

Your immune system evolved to destroy stuff that doesn't belong and leave the other stuff (self) alone. There is an immediate immune response. But it is also an adaptive system, so that it learns to leave cells alone that have been there a long time. That's why organ transplants don't require lifelong immune suppression. Cancer cells have to be sufficiently different from self cells to elicit an immune response. Prostate cancer doesn't get a huge immediate immune response and adaptation occurs quickly. Provenge amps up the immediate immune response, and it is hoped that checkpoint blockers (like Keytruda) will slow adaptation.

so how does Gleason grading come into this picture.

so would Grade 5 - or well differentiated PC cell - or cells that are much more different than normal cells - should get a better immune response than Grades 3/4 or Grade 5 means that they have adapted to have a better "invisible cloak".

So when there is a "immune" therapy - it would actually work better with the higher grade cells.

I don't know of any association between Gleason score and immune response.

Immune response...IMO...has a lot to do with answering the question: why do some people (and families) seem to remain cancer free, and others (and families) have cancer everywhere?

Yes, the presence of some gene or other is part of it...but the genetic code is ultimately responsible for our immune basics, is it not?
