Since you list perineal invasion as part of your diagnosis you might want to ask about
a bone scan. My PSA was 9.8 and as a G1o metastic spread was considered possible, if not likely. The bone scan showed the cancer had in fact metasticized to multiple places in my body. With that information I made the decision to skip the surgery and have IMRT instead. As someone else noted, why suffer the SE's from both of you don't have to?
The final decision as to direction can only be made by you but I would highly recommend that you get more testing and a second opinion from an RO before you make it. Whatever you decide, I wish you only the best.
