Should I Sling

I'm 71, now 2 years post robotic assisted prostatectomy. My PSA remains undetectable.

My urinary continence improved over the first year or 18 months but has stalled or gotten a bit worse since. I leak an average of 25mls/day with a low of maybe 5ml on a very quiet day and a high of 55 when i'm wrestling a grandchild.

I'm reasonably good at managing it with light pads for quiet days and larger ones for heavier days. Probably haven't wet through my outer pants for quite a while.

My motivation for doing something like the sling would be comfort and hygiene. I'm tired of feeling damp and chafed and having to hassle with supplies. The most bothersome time is probably TV watching in the evening and overnight where I either put up with a tight pad or damp nethers.

The downside is surgical and outcome risk. I did not enjoy the initial surgery and i wouldn't want the incontinence to get worse. I looked at an animation of the sling placement and it seems like it could go wrong either immediately or over time.

Any advice from guys with similar conditions?

If I were in your situation, I would sling.
I looked into it, but the Urologist said bc I have had radiation to the pelvic region it would NOT be a good idea.

Update: I saw the urologist today who would do the sling.
First I met with a (young, female - not that there's anything wrong with that) resident who was a bit discouraging. Not quite saying it but clearly believing that the leakage i was experiencing was not bad enough to warrant the intervention.

The doctor himself was much more positive, believing that the sling would definitely help and that the surgery was well tolerated. He said he had done many dozens of them, fewer in recent years because of better results from robotic surgery.

Next step is a cystoscopy in early january to see the lay of the land.

I decided to go with a clamp, and found a good one, but the source seems to have disappeared. (see my recent post started on that topic)

Whlie researching my options I got several opinions from 3 different urologists. One urologist was in NYC, and recommended by several different docs. He told me that the majority of his practice was removing slings from patients who had the procedure and were unhappy with it. That was over ten years ago, maybe things have changed, but it was enough to convince me to stay with the clamp. The one I use is fairly trouble free, and only occasionally uncomfortable.
