Bone mets and lower back pain

I’ve been lurking here for about 3.5 years dx in July of 2016. Long story short I was a G9, had a RALP, Radiation had to wait due to issues with emergency room screw up. Had radiation finally, then PSA started rising again.Doubling rate approximately every 28 days. Bone mets discovered Feb 2018. Back on Eligard, Zytiga, Xgeva, and prednisone. Everything is quiet now with PSA undetectable but I’m having lower back pain. Using CBD’s every day and OxyContin when the pain goes up. Usually after overexertion (washing the car, long motorcycle ride, etc.). Any recommendations for reducing the pain and reducing my opioid intake?

We'll need a bit more info for a better answer. What's your current PSA? Have you had any scans of your back to see what's up?

The question really is, "What's causing the pain?" If it's a PC met, then a bit of spot radiation should help.

There's a million things that can cause that pain so you'll likely want to see a back specialist at some point. Other possible things to do might be physical therapy or acupuncture.

and hate to say this, but it could just be age and doing too much ... maybe shorter rides and you can pay the cash wash folks ... just sayn'

With undetectable PSA I doubt any pain is coming from bone mets(although we naturally think that). I now know my back pain is from severe spinal stenosis and arthritis.

That is what I was told also... I have lower back pain, not real bad and was told with such a low PSA it isn't METS. I was told usually you feel it in middle and upper back if it was METS. I am seeing my Oncologist this Fri to discuss my situation for next steps...

Dale, How did you start to notice the pain from the bone mets?
Was it every now and then OR constant pain.
Was it initially mild pain or pretty excruciating pain.

I'm trying to prepare myself for the inevitable.

I have had sciatica before and I think this is the way it started. I’d have the hip pain and the jolts shooting down my leg. Now the pain is more in my tailbone and lower back. It’s not really bad but it has slowly gotten worse over the last few months. Guess I misspoke concerning me PSA. It is not undetectable. Here’s my levels for the last 8 tests:

12/19 - 8.59
11/21 - 6.10
11/7 - 6.93
10/24 - 7.43
10/9 - 11.5
09/26 - 9.16
08/29 - 8.60
08/20 - 8.12

I really thought it was just sciatica for a while so I can’t pinpoint the exact time it started. The pain hasn’t really increased a lot but it is more persistent. Nothing even approaching excruciating, just a persistent dull ache.

I work hard at staying upbeat about all this. I had a doctor tell me a few years back tell me “Your surgical results are about 50% the doctors and nurses and about 50% your attitude toward it”.

Wow that is a huge difference from undetectable. It's ok to ask questions here but I'd be talking to my MO asap. And depending on where you go perhaps line up a 2nd opinion.

Just got my latest results from my MO. My PSA dropped to 7.35. Hopefully just a flare from getting off the Casodex. Guess I should post my full story. I’ll work on that.

As an initial G9 with recurrence treatments and now elevated PSA it makes sense to me that bone Mets could be causing the pain. Others have had such experience at lower PSA. I am with GoBucks, get the MO on this and if there is not concern and investigation, I would be getting a different MO.
