Hello All , I have stage 4 , to 1 lymph node in the pelvic area . 5 yrs ago primary SBRT . All good till recently . PSA climbing . Had a PSMA-Pylarify scan at VA at Hines , picked up 1 hot lymph node in the pelvic area. . Also had a 6 month jab of Eligard Dec. 9th at VA . So I had a marker ( fidicual ) placed last week into the lymph node at Illinois Cyberknife , Dr. Hirsch . Went to planning a few days ago for scans .Initially Dr. Hirsch said 3 fractions of SBRT and 25 fractions of IMRt .At what greys I dont know yet. But Im scheduled to see DR. Szmulewits , ( MO ) at the University of Chicago this friday . So my question is , what are the must ask questions I must ask a MO on our first appt. Thank You

I saw my Uro and MO frequently until I signed up with my new RO for an ORIOLE-style treatment program. Salvage to the pelvic lymph nodes and spot to the 3 pelvic lymph gland mets found with the axumin PET scan. I had already had 70 Gy to the prostate bed. This round I got 50Gy to the pelvics and 56Gy to the mets for a total of 176 Gy (so far). Last 6 month Eligard injection was last June 5.

My RO said to stop all chems after the last radiation treatment (second week of last March) so I let the Eligard 'expire' this past Dec 5 when I was due for another injection. I informed my MO of this new strategy after I did his comprehensive blood panel + PSA (less than 0.1, an 'effective zero'). He agreed with the RO's plan, said he would see me in 1 year. I will be getting a PSA blood test every 3 months for 2 years, then to every 6 months. No more treatments or chems unless I flunk a blood test.

Thanks TrailGuy , I hope to stay on just eligard , but some say add Zytiga for 18-24 months . I too would like to see where the PSA goes after radiatio Much to think about . Thank You , JJ
