Radiation after Prostatectomy - PSA back to .09

I've been reading through posts most of the afternoon and have a plan, but would like to get some opinions before trying to get my doctor to go this route.
I am 56 & had prostatectomy on 3/15/22 - PSA 8.3, Gleason 9, 1 lymph node positive out of 21 removed, seminal vesicle involvement. Decipher score of .93 (very high risk). I had a PSMA Pet scan back in February (part of clinical trial) that show only my lymph node and prostate as positive. I had a very difficult 2 weeks after RP with extreme pain in perineal area.. gradually recovered and back to work and handling 14 hour work days by June.
6 week PSA = .09
3 month PSA = .055
4.5 month PSA = .09 (last Friday)
So just about all my data points to high or even very high risk of BCR.

I have an appointment with my doctor on Thursday and from what I've read I want to throw the book at it now.
Immediately start Orgovyx, then in about 2 weeks start RT on prostate bed. Seems like I should stay on Orgovyx for a year or so. Trying not to get hormone therapy resistant but also give the cancer a good shot of low low testosterone levels.

According to Dr. Eugene Kwon at Mayo, you have a 33% chance the cancer is in the regional area. He supports not blindly radiating until you are sure where the cancer is.

According to Dr. Anthony DiAmico at Dana Farber, if you don’t radiate know, you may miss the chance for a cure.

As mentioned, waiting another 6 weeks to see what your PSA is doing is an option. I suspect this is what your doctor will recommend.

I definitely had a slower than expected recovery from RP. 2 weeks of severe pain, horrible bruising from waist on down, doctor was shocked. Took me about 2 months to get back to working full-time and I'm a very active and in shape person. But the last 2 months I feel nearly back to 100%. Still have slight incontinence and ED. So I'm thinking by the time I get the radiation scheduled and ready it will be another month and I will be nearly 6 months post RP.
My concern is the longer I wait the lower chance I have of zapping the cancer in the prostate bed and more will be metastasized. I also realize with my high risk numbers I have low chance for cure, but it just seems the longer I wait the worse my odds. Also thinking if I really have horrible side effects from radiation or ADT, I can back off.
Sound reasonable or am I jumping the gun?

Dave - I had terrible gas pains in my shoulders for a few days after surgery and also a lot of bruising. Took a long time to feel right but you get there eventually as it occurs over months not weeks for many men. I was pretty good at 4 months and flew out west for a road trip in OR & CA.

However, at 5 months I went with adjuvant radiation which killed a couple of months with minor side effects other than tiredness near the end and for a while after. I did have some urgency after RT was over but it resolved pretty quickly as I got used to everything. ART pretty much finished off the ED as the doctors indicated might happen so that's how it goes when going after the "durable remission" or "cure" to some people. In my case, incontinence was not a problem and ED is just not that important with a wife with similar cancer issues.

When my RP failed I threw the kitchen sink at it — opted for full pelvic radiation and 6 months of Lupron (I was assuming the cancer was either in the prostate bed or lymph nodes). 3 years and four months of <0.01 readings so far and hoping for many more.

My husband's situation is a little different because he had 5+ years between his surgery and when his PSA became detectable. But the issue is the same: when to pull the trigger and, when you do pull it, what will it entail.

To help us with the treatment plan, we saw his local doctors, but also got second opinions from Sloan Kettering and UCSF. Both those doctors said that the triple play (in this case radiation to the prostate bed + radiation to the pelvic lymph nodes + hormone treatment) would lead to the best results, so that's what we did.

My husband finished his salvage radiation in March. He's been on Lupron since January, and will probably be on it for 12 to 18 months total. We'll know more in about two years but, for now, we're hoping for a durable remission.
