Thats quite a jump, and i think (Not an expert) it is the type of jump you would see with prostatitis. But i am not a doctor, and you know you have some cancer in there. So be vigilant and work with your uro. Don't know much about the effect of Tamulosin

Your upcoming appointment is timely.

Get out a notebook and start writing down essential questions.

It's easy to get sidetracked during those brief appointments, when we know our doctors are busy.

If you have urgent concerns, you could send a couple of questions today through your online patient portal.

Some questions that might be pertinent:

* Can I start a round of antibiotics now, such as CIPRO, to see if it has an an impact on a possible case of prostatitis? (Actually, this IS a question that could be addressed today, before your next visit!)

* Do you detect irregularities in my DRE examinations?

* Do I need an updated biopsy?

I'm glad you have an appointment soon. Take time to methodically prepare your questions and bring them with you.

I always state "I have 5 questions to address before we part today, Doc." I state the number of questions in advance ... that way, you know you will get all your ground covered.

I see my doctor every month, so I usually just have 1 or 2 main questions each time.

Something else I do with my doctor ~ we take a moment to discuss what we will look at and discuss NEXT TIME. I did this every day in my classroom ~ briefly touching upon the next day's lesson.

It kept everyone on the same page and set a framework for the future.

My doctor and I do the same. It leads to top-notch consultations and makes me an equal *partner" in the process!

I would reach out to your doctor today, since you have recent rises in PSA, and get a prescription for antibiotics going, to see what impact it may have on your PSA levels.

Everything to gain ... nothing to lose ... a proactive step before your next appointment.

Caffeine and alcohol can impact urinary function and urgency, as well ~ as you likely know.

Stay vigilant, stay proactive ~ as you have been doing!

Does anyone know whether a low grade pca can mutate to an aggressive form of pca by taking Tamsulosin or any other meds?

I've was able to get the prescription for cipro from my GP doctor today so I'll give a go and hope for the best before seeing my Uro on Sept 20th

I've now being on 3 days of Cipro 2 pills per day and my urinary flow has increased dramatically (my prostate is 85cc's) I can't believe the improvement, getting up once per night to pee, would that be a sign that I had prostatitis or an infection that caused my psa to jump so suddenly? I'm keeping my fingers x'd that my next psa before I see my Uro on Sept 20th that it's below 10ng/ml like it has been for the last 11yrs I've been on AS, can Cipro work so quickly?

Saw my Uro yesterday, did the dre and said it felt fine although it's very large, said for me to finish up with the 2 week supply of Cipro and get another Psa in 4 weeks time, if the psa hasn't subsided then another mri or biopsy is in order.
Just wondering if there's an expiry date for AS or have people been known to stay on it for the rest of their lives or eventually treatment is in order

You obviously had some infection, as your PSA is somewhat lower after taking antibiotics.

That being said, you stated you did have traces of Gleason 6 prostate cancer, and have stayed on a course of active surveillance for over a decade.

You've seen an increase in PSA over time, and your last biopsy was almost a decade ago ~ in 2013.

Prostate can mutate and evolve.

I feel, that it's maybe time for a new biopsy.

It's obviously on your mind, and I think part of active surveillance includes updating one's biopsy ~ after almost a decade ~ since there were traces of cancer there a decade ago.

You want to be ASSURED that active surveillance still fits your current situation.

A biopsy would give you that peace of mind, since it was last done almost a decade ago.

If your PSA is high because of an infection, it should come down. But you should be mindful of just how much it does. The better (stricter) AS programs advise men with a PSA over 10 to seek treatment for their PCa.

If you become fairy sure that your PSA is, in fact, going to surpass 10 in the near future, you may want to consider treatment before it does. You want to avoid this situation: your PSA rises and you have another biopsy that finds non-indolent or even high-grade cancer. You will be second guessing yourself as to when these lesions formed and whether earlier action on your part might have prevented the disease progression.

It is true that many men can stay on AS for their entire lives and avoid ever having to treat their cancer. But if your PSA is trending upward, you may well be advised to seek treatment even if your biopsies remain G6 (3+3) because your tumor burden (number, size, and location of lesions) exceed the guidelines of your AS protocol.

I had a 20-year history of BPH which led to many (negative) biopsies whenever my PSA rose a bit more than what my uro though could be attributed to my BPH. My last biopsy came back with G10(5+5) cancer. The moral is that you can have BPH and prostate cancer.
