I thought I would share my incontinence treatment following RARP. Initially I had moderate incontinence with a lot of frequency and nocturia resistant to all oral medication. The incontinence improved a lot with physical therapy but the frequency and nocturia did not. Incontinence got worse following following SRT. My flow also slowed. Flomax worsened incontinence without improving flow. Cystoscopy showed some urinary retention but no evidence of stricture or obstruction to flow. My urologist could not explain the physiology since I had no prostate to cause obstruction and I really had no flow problems before surgery. We tried 100 units of botox, injected in the OR. That help the frequency but I had to strain some to void. 6 months later I had another injection of 200 units of botox. Mild "squirts" of incontinence especially in the evening persist necessitating a small pad but no nocturia. However, I really need to strain to void. Overall better but not perfect. Probably will go back to 100 units every 6 months. FWIW in case anyone is considering botox
