Hanging in there with mCRPC

I’m coming up on eight years with Stage IVb, I’ve been fighting the disease with virtually every medication there is for this.

Part of the fighting is in my head. Staying positive, though last year was tough PSA spiked to 50. Cabazitaxel only reduced it to 38.

I’ve accepted how this is going to end. I sometimes get angry at the cancer and think “You SOB; You may win in the end it it’s not going to be easy.” I have no basis for this but I actually believe this “anger” at the cancer has a positive effect.

I arranged my affairs assuming I would not survive 2025. But then came PLUVICTO, started 9/2/2024. In just ten daysmy PSA went down 32% in just ten days. Just got Treatment #2 and my PSA is 75% down from Start. AND PSA is highly prognostic.

I feel PLUVICTO represents an entirely new category of weaponry, in the treatment arsenal.

You have already had some significant decreases in PSA levels. Obviously, your first couple of treatments have been IMPACTFUL.

PLUVICTO (Lutetium-177) is starting to become more mainstream ~ I now see commercials for it on television, even.

I’’ve been reading about PSA prognostic. Until recently they were assessing prognostic by PSA drop at 16 weeks. After at least one Clinical trial it’s been accepted that by four weeks you can get info on efficacy.

If you reach PSA30 [30% reduction] at four weeks you have a good prognostic. This is important since if it’s not working you can change treatments sooner.

The Gold Standard is to ultimately reach PSA90 [90% reduction]. Statistics are markedly better than even PSA80.

I had accepted that there was a good chance I would not survive 2025.

I ended up with a striking response to Pluvicto. I hit PSA30 in 10 days. Before my second treatment I was PSA75.

It’s hard to convey how happy I am. You mentally go from “I’m not going to survive next year” to “hey, I’ve still got some time”. It’s like the expression “I’ve got a new lease on life”.

Hang in there and fight. One of our Members noted he was playing “Kick the Can”. It’s a fair explanation of what I’ve been doing the last eight years.
