Pluvicto has show some remarkable results. It attacks cancer cells that exhibit PSMA. However, there are always residual cancer cells that don’t produce PSMA. I don’t see any discussion of going after those cells promptly.
I’m wondering: why are we waiting for the beneficial effect of Pluvicto to dissipate and not immediately going after the residual cells with a different treatment whose target is not limited to PSMA?
It seems: You get Pluvicto; if you’re lucky and get a great response your OS could be two years or maybe more but the cancer WILL be back. The seeds are still there.
Why are we waiting for progression to move to another med. It seems we’re just waiting for the non-PSMA cells to get up a head of steam, which they will, and then going after them?

The only "broad spectrum" cancer drug I can think of is chemotherapy and usually that has been tried before Pluvicto or equiv. is considered.
The concept is valid but the current strategy/SOP seems to be to exhaust ADT methods prior to moving to next steps with castration resistant treatments and chemo tends to be the next step. My wife takes a hormone med in addition to oral chemo but that is because the cancer is still hormone sensitive per pathology and BCa does not have something like PSA to monitor progress so they hit it hard and hope for the best given limited alternatives (btw, she is doing well @ 4 years)
I am just guessing that they are getting Pluvicto and equiv. dialed in regarding dosage, efficacy, and side effects where adjuvant treatments would complicate the monitoring and might cause injury or death of a patient. This is were the trials come in and I suspect that there may be some underway or soon to happen.
It is a little like getting PSMA scans at the diagnosis stage which is far from routine. Makes a lot of sense to most men to try everything possible to locate then destroy the cancer that can be found rather than the whack-a-mole process we tend to follow.

I believe (!) that more chemo is possible after Pluvicto failure. Not only docetaxel but also cabazitaxel.

Getting Pluvicto up front before chemo is now a thing. So perhaps those guys can get a chemo follow-up after Pluvicto.

Yes all of this energy around psma.
There are immunotherapy trials going after another target called Steap1.
