I have had this issue. My AST and ALT (especially ALT) started increasing right before my first Remicade infusion, and continued to increase with subsequent treatments. I've been on Remicade since April 2017, with monthly infusions since October.
However, both autoimmune hepatitis and primary sclerosing cholangitis can occur with IBD, so it's important to investigate the cause if your levels continue to climb.
They did a full panel of labs first, and when certain autoimmune markers were abnormal, they ordered a liver biopsy. That came back with nonspecific inflammation, but no evidence of AIH or PSC.
My AST and ALT recently climbed again this week (64 and 112, respectively). We are following it closely, and my IBD specialist has ordered an MRCP just to be safe. Also, my specialist said they sometimes see patients whose liver enzymes climb in parallel to intestinal inflammation.
Increases in liver enzymes can happen with any number of drugs, including prednisone, Remicade, Humira, Entyvio, etc., but since there are liver diseases that we're more predisposed to, like AIH and PSC, you should get checked out, particularly if you're seeing jaundice.
