Mesalazine suppository or Budesonide foam for 4cm inflammation?

Hi, I've been on 1g nightly Salofalk suppository for over 6 months now + morning budesonide foam trying to treat the bleeding with BM's.

I read on here that perhaps the budesonide foam is bypassing the inflammation and it would be better with the suppository, but again, I do take that every night and still bleed.

I am scheduled to meet with the GI who prescribes the biologics on Thursday, and I'm just freaking out that maybe Im missing something or haven't tried something...should I stop the foam and take 2 salofalk, what do you reckon?

it's quite possible that you are doing the right things, but the source may be further up the colon -

honestly tho, i don't have any experience with either of the rectal meds you are using -

my thought is keep doing what you are doing till you see the doc -

what is your diet like now? i know you've mentioned some changes - just know that progress can be slow for a lot of changes, so patience is important -

Suppositories do little for me, foam works best.

No it doesn't bypass the rectal inflammation (as per the actual measurement of the nozzle with the anal canal). The foam would cover the rectum, up into and possibly past the sigmoid.
Your dosage of suppositories is low...can you try the 4g enemas?

Im not sure. Im on 1.200mg x 4 oral dose daily (Mesavant).

Since my inflammation is limited to last 4cm wouldn't the suppository be more ideal even at a loser dose of 1g vs 4g retention that would be more spread out?

I agree with quincy that the mesalamine enemas would be worth a try. I did them nightly for many years along with the max dose of oral mesalamine (4800 mg a day). I don't need them anymore while on xeljanz.

When I was first diagnosed and In the hospital, they gave me 4 mesalamine suppositories a night. I don’t know if that’s an option? Never really heard about it being done afterwards. Not sure if it was because I was so sick at the time and to jumpstart my treatment? Thoughts anyone?
