Treatment for Proctitis - Ileorectal Anastamosis

Hello fellow UC sufferers:

I had ileorectal anastamosis surgery completed 11 years ago for UC, so my small intestine was joined to a small portion of colon and rectum that was left behind. This improved my quality of life significantly - 80% to 90% - but I still have rectal inflammation (proctitis).

I've tried Hydrocortisone suppositories, Uceris, and Canasa suppositories, but none of those work. Before I had surgery, I tried a variety of mesalamine medications which were also ineffective.

Any suggestions of UC medications that worn especially well in the rectum? With all of the nee biologics now available and TNF inhibitors, I've been curious to try something new, but I don't know where to start.

Thank you. How long were you taking Xeljanz before you noticed improvement?

Xeljanz is one of the fastest working medications. Many people see changes within a week. It is associated with a slightly increased risk of thrombosis and some other heart/circulation problems, although it's not clear if that's applicable for UC patients or only to RA patients over 50 where the data comes from (it was licensed for RA 5 years before it was licensed for UC). I have been taking xeljanz for 3.5 years and had no side effects that I'm aware of, but I'm 40 and very active.
Xeljanz belongs to the JAK-inhibitor group of drugs. They are not biologics and you take them as tablets. Some of the newer JAKs (e.g. rinvoq, jyseleca) are more targeted than xeljanz and as a result may be both safer and more effective.

I also had success with Xeljanz. I was I remission (zero symptoms) 9 weeks after starting it and off prednisone about 4 weeks after starting with some mild symptoms still.
