i have proctitis, 'severe' and 'stubborn.' It's usually quite difficult for me. I tried a number of immuno-suppressants (salazopyrin, methotrexate, imuran) and i am currently on 6mp at the highest dose for my weight. You might have some options in another immuno-suppressant. I also take mesalazine orally, daily.
When i feel/sense a flare, i jump on the topicals, pentasa suppository or salofalk enemas. These can sometimes seem to be of no help, but keep putting them in, they can be hard to retain on an inflamed bowel, even when empty. It's my understanding that the biologics work better for crohns than uc.
Other things which might have sent me into remission are, probiotics (dairy free, the type u keep in the fridge or vsl#3), vit d supplement, eliminating grain, dairy, sugar. Eating fermented foods often, sauerkraut, seeing a chiro and finishing my showers in cold water from chin down. Also, had a course of acupuncture.
all the best.
