I wish I knew. There is always bacterial translocation going on to some extent,along with endotoxins. It is also known that the mucus barrier is thinnest distally, and
that protein fermentation takes place distially,mechanical abraison might also be a factor,don't know. A million other things might be going on,such as mucus eating or degrading bacteria,thin mucus, not enough mucus.
