I would think it's rare. They leave only a tiny cuff to connect to the j-pouch - a few centimeters, and mine was stripped of cells. The pouch itself is created out of the terminal ileum. I asked my surgeon about small bowel cancer, and he said it's extremely rare. The inflammation should be gone too even in the cuff, unless you have cuffitis, or pouchitis, you're no longer dealing with chronic inflammation which is what increases the chances of getting cancer for UCers. If you have inflammation post j-pouch surgery, cuffitis or pouchitis frequently, perhaps you would be at risk.
