Ditto how do you feel on those recommendations? Certainly the emotional aspect is big. I try to be pragmatic about it all, measure the options left and their relative chance of success.

You've had UC since 2005, during that timeframe how long has your UC been in a remission? I ask as CRC odds rise by the decade, more so if you have not been well managed and have had predominately uncontrolled chronic inflammation. I'd opt for the colectomy if just nothing has worked in your history.

Are there any biologics left you haven't: stelara or xeljanz? I'd go for the xeljanz before I went into a clinical trial for another JAK-inhibitor (another flavor/brand competing with xeljanz). As straight xeljanz you know you are getting the real med and not a placebo. How do you feel about the biologic/trial options left?

Does the Chrons/colitis mean you're likely to need an end-ileo with an appliance? Not sure if you're a j-pouch candidate or not. How do you feel about the surgery options available to you?

If it is UC then I'd suggest surgery. Think about your quality of life. My CD/pouchitis is much easier to treat now that my colon has been removed and I have a j-pouch.
