A 2-week course of Cortenemas will do a lot for moderate proctosigmoiditis, which you seemed not to have, according to your past reports. It would help upper descending colon if you stayed on it longer, at least for 4 weeks-- by systemic effect. I don't think you'll have side effects once you get over the scope & prep irritation. Steroid meds tend to increase one's appetite. Be sure to lube the nozzle well when you insert the Cortenema. If your test results come back again this time with descending colon as the main inflamed part, and you can't tolerate oral mesalamine, then you really need to get a 2nd opinion from a gastroenterologist more experienced in treating & medicating IBD. From what you've already told us, it doesn't seem like your present doctor has other strategies than putting you on steroids periodically, which won't get you to remission for very long, and you don't want to risk the longterm effects of repeating steroids. Did the scope include a look at your terminal ileum? / Old Hat (40 yrs with left-sided UC; presently in remission taking brandname Colazal)
