Help with scope results. Awaiting biopsies

Hello everyone,

I just had my colonoscopy. I’m lucky to be at the every 2 year marker as I’ve be diagnosed since 2003. Yay me …

My results were as follows:

“All results appeared normal. All mucosa normal until 5 cm of rectum of very mild proctitis. Granular appearing mucosa.”

No mention of friability.

I used the pico salax prep. I’ve used this twice before and not had any prep related redness or irritation. I did feel this prep was harder on my system. Maybe my age or the food I ate days leading up.

Anyways, there were biopsies taken of all the normal appearing areas as well as the 5cm area that looked like inflammation. So I will know in a couple weeks I suppose.

What does “very mild proctitis with granular appearing mucosa” mean?

Regardless, it’s been prescribed to increase my enemas to every night for at least a month. Eventually I will taper back to my maintenance of twice a week. I will also be incorporating the salofalk suppositories in the morning. If this is mild proctitis I would like to hit it hard early.

Any ideas about if this is possibly caused from the prep? And what mild granular appearing mucosa means?

Granular does seem to mean inflammation...so good timing for the scope. The biopsies will determine severity.
Good to increase meds.

Did he see a hemmie...to account for previous bleeding?

Hey Quincy, thanks for the reply.

He did say I had hemorrhoids but they were not significant or big. He did not say whether they were to blame for the blood on my stool but that it was the most probable considering it happened inconsistently and when stool was harder. I still really have no idea though.

RE your Q about prep possibly affecting scope report: from personal experience I think the prep can definitely revv up internal hemmies. That happened to me right before my last C-scope in 2019; they kicked up a fuss & started to spurt fluid as I was finishing the last of the prep, Miralax mixed with plain Pedialyte. Luckily that didn't impact on my biopsy report so I hope yours also turns out ok. My longterm remission 5-ASA dosing conquered the spurt within a couple days post-scope, whew! That had never happened to me previously when scoped during remission & using that same prep so I'm not sure whether aging contributed to the issue, too, but my gastro didn't consider it a big deal. Good luck & take care; it sounds like you're on a pretty good track if not a perfect one. / Old Hat (40+ yrs with left-sided UC; in remission taking Colazal)

Hi Old Hat,

Thanks for the response and your experience. I’ve upped my enema’s to nightly already and am deciding whether to add in a suppository in the morning for good measure since he says the inflammation he sees is 5cm right at the end.

I am waiting on the biopsies but I’m pretty certain this is a mild flare. He sees so many colons he must know what he’s looking at I would think.

Hi everyone,

I just wanted to come on and post a quick update in case anyone is searching this topic. I just got my biopsy results. Everything was clear of any architectural changes and inflammation except the very bottom of my rectum. Last 5 cm I had “chronic active colitis”.

My plan is to continue with nightly salafalk enemas for at least a month then reevaluate. Hopefully at that point I will start to taper to every other night. I am also adding a morning salofalk suppository for at least 2 to 3 weeks and really evaluate at that point. I may also then start to taper that if all is well.

My symptoms have stayed the same and have not changed since my scope and so I am hopeful that this will work.

Sounds like a very mild and limited Proctitis flare and your mesalamine regiment sounds appropriate.

What does “very mild proctitis with granular appearing mucosa” mean?

Proctitis is rectal inflammation. Granular mucosa is suggestive of mild inflammation. It looks rough in texture like sandpaper. Normally the intestinal surface is smooth, like the inside of your mouth or back of your throat.

Overall a pretty good scope and report. Remission is always best, but mild and limited inflammation is a second place.
