Need help from someone who may know about radiology conclusions. My surgeons office and I keep playing phone tag. I am almost a year postoperative with a toupet and hiatal hernia fix. The toupet was recommended by my surgeon (cardio thoracic) due to failing the manometry swallows and slow gastric emptying.
After the surgery I still had my same symptoms, chest tightness and chest pain, seem worse post op. In July 2016, EGD said wrap was good but still showed esophagitis and Bravo probe showed NO acid. He thought it was my esophagus. We tried various meds such as calcium channel blocker, nitrate, theophylline, and Baclofen. The Baclofen worked great but only worked for 5 months and then overnight it seemed to stop working, if that is at all possible? What is strange is I feel better chewing gum, actively eating, or lying down. It's now back to testings and Appts. Tried a PPI for a month and did not help. My surgeon ordered another Esophagram and my PCP had me go back to the Cardiologist and he ordered a CTA heart (I'm a fit 43 year old female and 2 years ago when this started I had a stress tests, EKG, and Echo....all good).
2/10/17 Esophagram stated:
1. Status post fundoplication without dehiscence.
2. Mild to moderate tertiary contractions of the esophagus.
3. Slight delay in transit of contrast across the GE junction in the semi prone position which clears with upright positioning, but no spontaneous or inducible gastroesophageal reflux.
2/10/17 CTA Heart:
1. Calcium score zero. Normal CTA.
2. Post fundoplication with mild distal esophageal wall thickening. Correlate for reflux esophagitis.
The two seem conflicting as far as reflux and not sure where the motility issues come in, as I feel like I eat fine. The chest pain/tightness is running my life and I thought the fundoplication was going to fix this. I know the heart is fine, but not sure why I have my same symptoms after fundoplication almost a year later.
