Nissen (or its varieties like Toupet) works well for most people, if they have a problem with the lower esophageal sphincter not closing properly. It is the most common and longest tested surgery for that defect. Is that your problem?
If you also have a large hiatal hernia Nissen is about the only thing you can get because most other treatments don't address the hernia. If you only have a big hernia and the valve is working you may not need the Nissen, just get the diaphragm (where the hole is) sewed tighter.
Are you taking your PPIs correctly? Have you tried other brands? Is the dose the prescription strength or the over-the-counter variety?
What is your problem? Are you seeing a real gastroenterologist, or your family doctor? Have you had all the testing? I especially like the barium swallow because it shows if reflux is happening while you are awake and in different positions and if you have a hiatal hernia.
